Social prescribing in West London
There are currently over 70 Social Prescribing Programmes and more than 450 Social Prescribing Link Workers in London. Services are managed by GP Practices, Primary Care Networks, GP Federations, and the Voluntary, Community and Social Enterprise (VCSE) sector.
The London Social Prescribing Map aims to support visibility of service models, key contacts for networking and good practice sharing across London.

Description of the Social Prescribing service
Hammersmith & Fulham Links Service started at the end of 2019 and currently has 12 Link Workers. The Link Workers are mostly employed by the Hammersmith & Fulham GP Federation and some are employed directly by the PCNs. The Links Service is managed by the Programme Support Manager.

We are a Housing Association with around 38000 properties. We don’t just build, manage and sell homes. We do our best to create vibrant communities. We reinvest all of our profit into creating new homes, maintaining our properties and supporting communities. Workshops to develop skills and help people find good jobs. Personal support for older and more vulnerable residents and activities for all ages. Social prescribing a great way to connect those most in need with the support services that we offer and to help us establish gaps in support needs.

Description of the Social Prescribing service
Enable provides social prescribing services to 5 PCNs in Wandsworth, as well as in Adult Social Care. The service includes a young person's social presriber and a cancer link worker, in additional to adult social prescribers. Our service started in November 2019, Enable were commissioned by Wandsworth CCG to deliver social prescribing to 7 out of 9 PCN in the borough. The CCG also commissioned Enable to manage a social prescribing capacity building fund to support the VCS. In 2020 Wandsworth Borough Council provided funding to employ 2 additional Link Workers who are based in adult social care. The council now also contribute to the VCS capacity building fund. In 2020 we were commissioned by Macmillan Cancer Support to provide a Cancer Community Link Worker who is embedded within the service. In addition, one of our PCNs have a Young Persons Link Worker who works with 15-21 year olds.

Description of the Social Prescribing service
Started in 2019, we operate a bespoke service in each PCN, we find that a one-size fits all approach doesn't work across boroughs. We take into consideration our PCN's needs and provide bespoke interventions to address local health inequalities. We encourage our SPLWs to use their knowledge and experience e.g. SPLW who is a Nutritionist supports patients to make a meal plan, anther SPLW does jabs in one of the vaccination centres.

Description of the Social Prescribing service
Our Social Prescribing service went live in November 2019. We accept our referrals directly from GP surgeries for patients over the age of 18 with evidence of social and wellbeing needs. Throughout the pandemic we worked closely with shielding and vulnerable patients as well as supporting the vaccine roll out in the borough. Currently we are working out how we can work in the 'post covid' world and working closely with the PCNs to reach patients who may have previously fallen through the gap when it comes to social support.
